Player Registration and Eligibility 

NEW – MSHSL CONCUSSION REGISTRATION FORM MUST BE FILLED OUT AND SIGNED BEFORE PARTICIPATION

THE FOLLOWING FORMS NEED TO BE COMPLETED BEFORE A PLAYER WILL BE ALLOWED TO PARTICIPATE.

• MSHSL Eligibility Form – Should be available the first week of August.
• Player Physical on file before the 1st day of practice
• Player Emergency Card
• Pay Player Participation Fee
